This isn’t a rushed evaluation or a quick adjustment.
Your Rehab to Performance Session is a comprehensive 75-minute one-on-one appointment with a Doctor of Physical Therapy designed to uncover the root cause of your issue AND begin treatment immediately.
During your session, we will:
This longer first session allows us to fully understand your body, your goals, and your history so we can create a plan that actually works.
Ongoing care plans are customized based on your goals, condition complexity, and desired timeline.
Most clients choose a structured treatment plan that lowers the cost of ongoing care while creating faster, longer-lasting results.
Clients who begin care within 24 hours of their first session receive a $250 Credit toward their treatment program.
Your nutrition should support the life and body you’re trying to build.

Build strength, confidence, resilience, and long-term athleticism with expert-guided coaching designed around your goals and your body.

Unlike generic personal training, our coaching is integrated within the larger Chain Effect system — allowing clients to seamlessly transition between rehab, performance care, nutrition, and ongoing training support as their goals evolve.
This creates a safer, smarter, and more sustainable approach to long-term strength, movement, and performance.
Package options and flexible session lengths are available based on your goals and level of support needed.
Long story short, if you have Medicare or Medicaid, we welcome you to use all the services at Chain Effect, however, we will not be able to provide you a superbill or documentation to get reimbursement after your appointment.
Here’s Why:
At Chain Effect, we believe everyone deserves access to top-quality care — including our Medicare/Medicaid clients.
Because we’ve chosen to stay “non-participating” with Medicare/Medicaid, there are some important rules we have to follow:
We can’t provide or bill Medicare/Medicaid for services that Medicare/Medicaid considers “covered” or “medically necessary.”
That usually means care meant to treat or fix a specific injury, illness, or medical condition.
We can work with you on things Medicare/Medicaid doesn’t cover — like wellness, performance, strength, recovery, or mobility sessions designed to help you stay active, strong, and independent.
These visits focus on prevention and performance rather than medical treatment.
Dry needling and certain advanced manual techniques are also not covered by Medicare/Medicaid, so we offer them only as self-pay services.
Because these visits are not covered under Medicare’s rules, we do not submit claims or provide superbills — even if you have a secondary insurance plan. Payment is made directly to Chain Effect at the time of your visit.
Our goal is to make sure you still have access to expert care — focused on keeping you moving, performing, and living at your best, even outside of the Medicare/Medicaid system.
